Here’s a step-by-step guide on how to obtain a Polio Vaccination Card online in Pakistan:
Step 1: Visit any government hospital or medical facility to receive two drops of the Polio Vaccine.
Step 2: After receiving the vaccine, visit the National Immunization Management System portal https://nims.nadra.gov.pk.
Step 3: On the portal, select the “Other Vaccination” option and choose “Polio” or “Yellow Fever.”
Step 4: Fill in all the required fields, including your CNIC Number (Computerized National Identity Card), ID Card Issue Date, and select “Vaccine” (Oral Polio Vaccine).
Step 5: Add two values as instructed and proceed by clicking “verify.”
Step 6: On the next page, complete the form with your name as it appears on your CNIC or Passport, enter your Passport Number, select your Nationality, and click “verify.”
Step 7: Make a payment of Rs100 as a one-time fee and continue.
Step 8: Verify the payment details and submit your application.
Step 9: Proceed to the “Certificate Download” button to download your Polio Vaccination Card or Certificate.
